No, there are no crafting failures in the game. Raising your skill level will allow you to craft more difficult items instead of increasing your success rate, as is the case in some other MMORPGs.

Yes. Blizzard is even planning on having seasonal fish -- and they can tie into quests and other trade skills. Players can also fish up things like locked boxes, unique armor, and weapons.
12.1 Will you be able to randomly find items, and if so, will these items be classified as in Diablo II (Rare, Unique, etc.)?
Monsters will have treasure that characters can loot, but these will be drawn randomly from a specific item class for that monster type. Blizzard do not expect to have the same vast amount of random items as in the Diablo series, but it will have items that are rare and even some that could be truly unique by limiting them to only one per server. Like Diablo II, World of Warcraft supports a form of random item generation. Unique items can be created by the system, such as "Sword of Lifestealing" or "Breastplate of Stamina." The game will take the base item, such as a sword, and randomly combine it with a modifier, such as lifestealing.
20.1 How does death work?
When you die, your body falls to the ground and you appear as a ghost in the nearest graveyard. You have three options for resurrection: you can be raised by a player with the resurrection skill and return to life with a temporary rez sickness, you can run back to your body and resurrect yourself, or you can ask a ghost NPC at the graveyard to resurrect you there for a small experience loss. You can raise yourself anywhere within about 40 yards of your body.
20.12 If I loose Xp enough that I drop below zero, what happens?
You enter negative experience points. You do not loose a level.
